Non-Binary Top Surgery: Designing a Chest That’s Yours

Top surgery does not have a single destination. Here is how non-binary patients can design a chest that reflects exactly who they are.

This article is general information and education — not medical advice. Everyone is different, and your surgeon or medical provider will know what’s right for your individual circumstances.

Top surgery is sometimes described as a single outcome, but it is far more flexible than that. For non-binary and gender-diverse patients, the goal is simply a chest that feels like yours — and there are many ways to get there. The conversation starts with your vision, not a template.

There is no single destination

Some people want a completely flat chest; others want a small amount of remaining tissue for a softer contour, or a particular nipple size and placement. All of these are valid goals, and the right technique is chosen to match the look you describe.

Tailoring the technique

Technique, the amount of tissue removed, and nipple choices can all be adjusted to your goals. Some patients choose nipple-sparing approaches, some prefer grafts, and some choose no nipples at all. Your surgeon’s role is to translate your vision into a surgical plan.

A consultation built around you

The most important part is being heard. Bring reference images and an honest description of what you want — an affirming surgeon will work with you to shape a result that fits, without assumptions about what your chest “should” look like.

The takeaway

Your chest, your design. Whatever your goals, they deserve a surgeon who listens and a plan made for you. A consultation is where that conversation begins.
